Custom fields not displaying in agent Form tab

This list has 20 fields, types HIDDEN, SELECT, TEXT, & RADIO, used to set initial campaign type filters & give the agents blanks to fill in. The list has 0 leads.
The list is active and attached to campaign TLA-0001.
Manual outbound calls work within the agent interface, and lead details on the main screen are saved/retrieved correctly.
On the form screen, I get ERROR: no custom list fields table.
