ADD new Callback(useronly or anyone) on Agent UI

Hi all
my enviroment :

Asterisk : 1.8.23.0-1_centos5.go
Vicidial : 2.12b0.5
VERSION: 2.12-493a
BUILD: 150703-2105
On Virtual Machine(DB+Telephony+WEB)

Manager asked me to create a campaign with 1 empty list that agents can populate it with manual dial.But requirements are :
We don't need to call in real time but we need to add a new lead without calling customers and then schedule a callback.
I'd like to create a link on Agent Interface that open a web page and agents can fill this form with all contact informations and permits to schedule a callback.After Submit this form I can pass all info to Non-Agent API to create a CB.

Do you have any tips?Any suggestion or someone has just worked on it?
I'm not a developer...
Could it works?
