i think this is asterisk error
you need to post to * forum.
looks like your provider negotiation problem
and i see bottom circuits busy .
check with provider or check your extension.conf again

FIXED... it was my mistake... i forgot to configure iax.conf properlly thanks ramindia for the advice... now i have this problem..

check how your provider accept the calls
i see the call going with 9 prefix
check with provider and remove prefix 9 if not required
